A modified totally stapled KONO-S anastomosis for ileal and ileocolic Crohn's disease: technical aspects and case series.

Questo studio, pubblicato su Updates in Surgery nel febbraio 2025, descrive una versione modificata della anastomosi Kono-S totalmente eseguita con suturatrice meccanica per il trattamento della malattia di Crohn (CD) ileale o ileo-colica.

La malattia di Crohn è una patologia infiammatoria cronica dell'intestino che, nonostante i progressi nelle terapie mediche, richiede spesso interventi chirurgici a causa delle recidive. L'anastomosi Kono-S, una tecnica chirurgica che prevede la creazione di un'ampia anastomosi anti-mesenterica, è stata sviluppata per ridurre le recidive post-operatorie, che spesso si verificano a livello dell'anastomosi o nel neo-ileo terminale.

Biosynthetic Mesh Reconstruction after Abdominoperineal Resection for Low Rectal Cancer: Cross Relation of Surgical Healing and Oncological Outcomes: A Multicentric Observational Study

The large perineal defect, with impaired wound healing and delayed start of the adjuvant chemotherapy, can make the reconstructive phase of abdominoperineal resection for low rectal cancer extremely challenging. Using biosynthetic mesh for the neo-perineum reconstruction after a Miles’ procedure is a poorly investigated technique, which, in our series, resulted in safe, reproducible results affected by limited complications. Moreover, for improved perineal wound healing, it guaranteed a faster start of the adjuvant therapy with clear reduction in oncological outcomes (i.e., recurrences and death).
